Do you have questions about Medicare and selecting a Golden Physicians Medical Group Doctor?
Please fill out this form to learn about your options and find a provider that is right for you.We completely understand that your health is one of the most important things in your life. Becoming a member of Golden Physicians Medical Group (GPMG) is easy, just follow these steps:
- Look in your health plan directory for GPMG.
- Once you have located your doctor and note the doctor’s name, location, and provider number and fill out the Insurance Plan Enrollment forms accordingly. You may also switch to a GPMG doctor by contacting your health plan and telling them that you would like to change to a GPMG provider.
- Or call our administrative office at (888) 909-0270 and we can have a representative walk you through the process with your health plan.
